IHN-CCO invites interested parties in Benton, Lincoln and Linn counties who can positively impact the health and well-being of IHN-CCO members to submit proposals. The selected proposals will be Delivery System Transformation (DST) projects that help transform the health care delivery system.
IHN-CCO is accepting proposals for innovative, transformational pilot projects that focus on Access to Affordable Housing, Inclusion, Diversity, Equity & Anti-Racism, Behavioral Health and Access to Quality Care.
Smaller capacity building applications are also being accepted for amounts of $15,000 or less. These funds will be prioritizing new partners and organizations led by priority populations that have been identified by the Oregon Health Authority. These include communities of color; tribal communities, including the nine federally recognized tribes of Oregon and other American Indian and Alaska Natives people; immigrant and refugee communities; migrant and seasonal farmworkers, individuals and families with low income, people with disabilities and LGBTQIA2S+ communities.
The deadline to submit a Letter of Intent has passed. Prospective pilot project applicants will be notified regarding whether or not they have been invited to submit a full pilot proposal no later than June 28, 2024.
Capacity Building project applicants will be notified of initial funding approval by the Delivery System Transformation Committee no later than June 28, 2024.

The full Request for Proposal (RFP) Guidelines are available above or by emailing [email protected]. This non-binding Letter of Intent is required to be considered for funding.

Transformation is the constant process of change. Encouraging transformation can happen in lots of ways, like creating new relationships, community partnerships, and connections outside of the traditional health services setting to improve community health and well-being.
Transformational projects look at health and well-being from an “upstream” point of view. This means taking on ideas that are focused on prevention, not just treatment, as well as breaking down barriers to improve health.
Being transformational and innovative also means taking a different approach and being willing to risk trying something new. Even projects that struggle or fail provide valuable learning opportunities. Transforming systems takes time and supporting innovative ideas now can lead the way towards a healthier future.

It is highly encouraged that potential pilot proposers attend the DST meetings to connect, network and learn about other transformation efforts in the community. The meetings occur every other Thursday from 4:30 to 6 p.m. both virtually on Zoom and in-person in Corvallis.
