As a member of the IHN-CCO, you have access to care coordination services.

What Is Care Coordination?

• Care coordination is how IHN-CCO makes sure all the benefits you have work together to support your health needs.
• You can have a care coordination team that will work with you to help you get the right care at the right time.
• The care coordination team can help you find doctors, understand your insurance and get services.
• They can help with non-medical needs, such as housing, food and transportation.

Role of a Care Coordinator:

• A care coordinator is a specially trained health professional.
• Their goal is to work with you and your health care providers. 
• Talking with your care coordinator is important for managing your health and well-being. 
• They can provide guidance and coordinate services.
• They can coordinate with other programs you are working with to make sure your needs are met.

Benefits of Having a Care Coordinator:

• Help with your health conditions and medications.
• Coordinate services and can connect you with community resources.
• Together with your care team, you can help develop a care plan. This plan outlines your health goals and the steps to achieve them.
• As your needs change, your care plan can be updated.
• You can have a copy of your care plan.

How to Get Started:

• If you have complex needs, a care coordinator will be assigned to you and reach out to you.
• If you are not assigned a Care Coordinator and you would like a care coordinator to help you manage a health need or develop a care plan, reach out to customer service.
