IHN-CCO Funding Streams

SHARE Initiative (Supporting Health for All Through Reinvestment)

IHN-CCO invites interested parties in Benton, Lincoln and Linn counties to submit proposals that support housing and/or increases the number of medical respite beds in the region. The approved proposal projects will receive funding through the SHARE Initiative January to December 2025.

SHARE is a way IHN-CCO supports social determinants of health projects and programs in Benton, Lincoln and Linn counties. SHARE is a funding mechanism with specific criteria from the Oregon Health Authority (OHA) and must fall into the categories of economic stability, education, neighborhood and built environment, and social and community health. IHN-CCO may narrow the scope beyond the contractual requirements.

SHARE Requirements

  1. Schedule and attend a Technical Assistance meeting with IHN-CCO Engagement & Transformation by Sept. 9, 2024. Please see the timeline below and connect to schedule a meeting no later than Sept. 12, 2024.
  2. Review the SHARE Request for Proposal Guidelines.
  3. Download and submit the following Proposal Application templates to [email protected]

2024 SHARE Timeline

  • Aug. 12: Request for Proposal announcement.
  • Sept. 9: Schedule a Technical Assistance Meeting by this date.
  • Aug. 26 – Sept. 12: Required Technical Assistance Meetings offered.
  • Sept. 15: Proposals due by 11:59 p.m.
  • Nov. 11: Community Advisory Council review and decisions.
  • Dec. 4: IHN-CCO Board of Directors review and decisions.
  • Early December: Proposers notified of denial or approval.

Delivery System Transformation Committee (DST)

The DST generally releases a request for proposal (RFP) annually in the spring. The DST is focused on innovative strategies that transform the healthcare delivery system through partnerships and collaboration with the ultimate goal of reducing healthcare costs, improving health outcomes of IHN-CCO members and increasing access to services. Provider Contracts With IHN-CCO

IHN-CCO is interested in hearing from community partners and providers on projects and programs in Benton, Lincoln and Linn counties that may positively impact the health outcomes of our members. There are multiple ways to connect with and partner with IHN-CCO including provider credentialing, Delivery System Transformation (DST) pilots, the SHARE Initiative and more.
