IHN-CCO’s Delivery System Transformation Committee Approves $1.7 Million in Funding for Innovative Projects November 24, 2025 InterCommunity Health Network Coordinated Care Organization’s (IHN-CCO) Delivery System Transformation (DST) Committee has approved more than $1.7 million to support 15 full-scale pilot projects and capacity-building efforts for five emerging community partnerships. These investments, which will launch in January 2026, reflect IHN-CCO’s ongoing commitment to improving health outcomes and advancing equity across Benton, Lincoln and Linn counties. The DST Committee includes community partners from health care, social services, housing, education and culturally specific organizations. Together, they work to champion innovative ideas that address health inequities while elevating community leadership. The committee is focused on supporting a health care system guided by community knowledge, rooted in shared values and strengthened through collaboration. Projects are selected from a competitive request for proposals process. Qualifying proposals must address at least one priority area from the 2024 Community Health Improvement Plan: access to quality care, affordable housing, behavioral health, or initiatives focused on inclusion, diversity, equity and anti-racism. The awarded projects are designed to increase access to services, reduce system strain and staff burnout, improve community well-being and help transform how care is delivered across the region. IHN-CCO has invested more than $28 million in more than 100 pilot projects over the past decade. These projects have strengthened local partnerships, expanded services and helped build a more equitable health system across the region.
