Community Health Improvement Plan (CHIP)

The 2019 Plan Is in Motion

The Community Advisory Council (CAC) for InterCommunity Health Network CCO (IHN-CCO) is tasked with overseeing a community health assessment and adopting a Community Health Improvement Plan (CHIP). This CHIP serves as a beginning place for planning for the improvement of IHN-CCO members’ health and quality of care while effectively managing costs.

The CHIP was updated in June 2019. It is based on the foundational work of the 2014 CHIP and its 2016 Addendum. The 2019 CHIP borrows from and builds upon the Regional Health Assessment and the three county Community Health Assessments of Benton, Lincoln and Linn counties.

Overview of Health Impact Areas & Outcomes

The 2019 Community Health Improvement Plan priorities are below:

  • Outcome 1: Increase the percentage of Members who receive appropriate care at the appropriate time and place.
  • Outcome 2: Increase the percentage of Members who receive care communicated in a way that ensures that they can understand and be understood by their care providers and that they are effectively engaged in their care.
  • Outcome 3: Improve integration of oral health services with behavioral and physical health services.

  • Outcome 1: Reduce stigma and increase community awareness that behavioral health issues are normal and widely experienced.
  • Outcome 2: Increase the behavioral health expertise of healthcare providers and staff to reduce stigma and improve access and appropriate utilization of services.
  • Outcome 3: Increase mental health and substance use screenings, services, referrals and peer and parent support.
  • Outcome 4: Improve care for Members experiencing mental health crisis.
  • Outcome 5: Improve care for Members experiencing severe and persistent mental illness.
  • Outcome 6: Behavioral health funded and practiced with equal value and priority as physical health.

  • Outcome 1: Increase the percentage of children, youth and families who are empowered in their health.
  • Outcome 2: Decrease child abuse and neglect rates.
  • Outcome 3: Increase breastfeeding initiation and duration rates.
  • Outcome 4: Increase integration of behavioral health and oral care as part of routine primary pediatric care.

  • Outcome 1: Increase the percentage of Members who are living a healthful lifestyle.
  • Outcome 2: Reduce the percentage of Members who use and/or are exposed to tobacco.
  • Outcome 3: Reduce sexually transmitted infection (STI) rates.

  • Outcome 1: Increase the percentage of Members who are living a healthful lifestyle.
  • Outcome 2: Reduce the percentage of Members who use and/or are exposed to tobacco.
  • Outcome 3: Reduce sexually transmitted infection (STI) rates.

  • Outcome 1: Increase the percentage of Members who have safe, accessible, affordable housing.
  • Outcome 2: Increase the percentage of Members who have access to affordable transportation.
  • Outcome 3: Increase the percentage of Members who have access to healthy food.
  • Outcome 4: Increase health equity.

The original Community Health Improvement Plan (CHIP) was published July 2014.
2014 CHIP

In February 2016, the Board of Directors for IHN-CCO approved an addendum to its CHIP. The addendum focuses on outcomes and indicators around these five areas: Access to healthcare, behavioral health, child health, chronic disease and maternal health.

The Regional Health Assessment (RHA) is a result of dedicated research and collaboration with community partners, agencies, leaders and local residents of Benton, Lincoln and Linn counties. The RHA looks at conditions and factors that affect people’s physical, mental and social well-being. The research conducted will act as a guide to inform decisions, prioritize health issues, and create plans and policies to improve community health in our region.2015 Regional Health Assessment
