IHN-CCO’s Delivery System Transformation Committee Approves $1.7 Million in Funding for Innovative Projects

InterCommunity Health Network Coordinated Care Organization’s (IHN-CCO’s) Delivery System Transformation Committee (DST) recently approved over $1.7 million in funding for 15 full-scale pilot projects and capacity support for 5 new or emerging partnerships at the DST. The committee is comprised of community partners representing a range of sectors, largely in social services and healthcare, with a focus on building and supporting transformational initiatives that address health inequities and center community needs. Anyone is welcome to join the DST that can have a positive impact on the health and wellbeing of our IHN-CCO members and greater community within the Benton, Lincoln, and Linn County region.

The DST committee envisions a healthcare system rooted in community knowledge, values, and innovation. The DST is committed to fostering an inclusive, engaged, collaborative space where innovative and transformational ideas are shared and supported with the intent of advancing health equity, centering community perspectives, and improving health outcomes for diverse communities.

Projects are selected from a competitive request for proposals process. The goal is to transform the health care delivery system while working to achieve better quality healthcare, lower costs, increase access to services, and reduce staff burnout. To be considered for funding, projects must address access to quality care, access to affordable housing, behavioral health, and/or inclusion, diversity, equity and anti-racism in alignment with the 2024 Community Health Improvement Plan, developed by IHN-CCO’s Community Advisory Council and the Partnership for Community Health.

Pilot projects will kick off their work in January 2026.

IHN-CCO is committed to improving the health of our communities by building on current resources and developing new partnerships. IHN-CCO funds and supports pilots that improve local health care in Benton, Lincoln and Linn counties. Over the last decade, IHN-CCO has funded over 100 pilot projects totaling over $28 million.
