As part of the Oregon Health Plan’s new Health Related Social Needs (HRSN) benefits to address climate, nutrition and housing needs, IHN-CCO will be granting Community Capacity Building Funds (CCBF) to community partners. In 2025 IHN will be awarding $3,601,581.00.
Proposals will be evaluated by IHN-CCO Staff and community partners not applying for funds with input from the IHN-CCO Community Advisory Council. Proposals will be evaluated for priority areas and appropriateness of funding to requirements. Review will occur in June and July 2025 with final funding decisions announced by late August 2025. Funds will be distributed by September 30, 2025.
Grants are intended for organizations to increase their capacity to engage with HRSN and the health care system through:
Partners will need to be able to contract with IHN-CCO as an HRSN Provider by working with IHN-CCO to obtain an Oregon Medicaid Provider ID, maintain an active business registry and liability insurance, as well as be willing to utilize web-based Community Information Exchange through Unite Us.
